Ursprünglich sollte die Scheibe "Metal up your ass" heissen - benannt nach dem Demotape von 1982 - und hatte ein anderes Coverartwork; eine Kloschüssel, aus der eine Hand mit einem Messer kam - heute bekannt vorallem durch Metallica T-Shirts und Aufnäher. Der Plattenfirma war das zu extrem und man verwarf die Idee - Cliff Burton sagte darauf "just fuckin' kill 'em all" - womit der Titel für das Album stand. Das Coverartwork wurde auch geändert, zeigte dann die auch etwas provokante Hand mit dem Hammer und dem Blutfleck.
